What is the site of a shoulder injection?
There are 3 potential sites for a shoulder injection:
- From the side: Subacromial injection are done from the side. This is the most common site for problems such as frozen shoulder, rotator cuff syndrome or bursitis
- From behind: The posterior approach is used most commonly for shoulder joint injections for arthritis.
- From the front: This is the least common approach but can be used for joint or soft tissue injections.
How effective are shoulder injections?
There is good evidence that shoulder injections have moderate to high efficacy in relieving pain and improving function for up to 2 months. They can be effective for longer but this varies on the situation.
What are possible shoulder injection side-effects?
Side-effects are generally minimal. The commonest ones are:
- Pain, swelling, or bruising at the injection site. This is generally minor and can last for a few hours for pain to days for bruising.
- Redness or flushing of the face. This is usually a transient phenomenon if it happens.
- Temporary increase in blood sugar or blood pressure. This can occur in diabetics or those suffering from high blood pressure. Usually the blood sugar or blood pressure can become slightly raised for up to a week.
- Insomnia or difficulty sleeping. This is difficult to predict but is uncommon.
- Thinning or discoloration of the skin around the injection site. This is uncommon for shoulder injections as they are deep injections that don’t affect the skin surface.

Can you have a shoulder injection in pregnancy?
It would be best to avoid shoulder injections in pregnancy and when breastfeeding. However, they may be given in certain circumstances and you should seek advice from your obstetrician before doing so.
What if a cortisone shot in the shoulder did not work?
A steroid shoulder injection should not be thought of as a cure for shoulder pain. A shoulder injection is meant to reduce pain and improve function to allow you to exercise your shoulder in order to lead to recovery.
